Making an Appointment
You or your doctor may call the CFA Center Monday through Friday at (650) 497-8265 to arrange for an appointment, which will be scheduled on a Wednesday morning. Please notify your doctor, dentist or other professionals involved in your child's care about your upcoming visit. They may have important information for our staff and may also want to be informed of your child's progress.
If you are coming to the center from a considerable distance, you may need to stay in the Palo Alto area overnight. A staff member can provide information on local lodging or make reservations for you.

Medical Records
Please arrange for copies of your child's medical records, dental or orthodontic records, X-rays, and speech pathology or audiology reports (if available) to be sent to the CFA Center before your appointment. All correspondence and records should be addressed to Craniofacial Anomalies Center, Lucile Packard Children's Hospital, 725 Welch Road, Palo Alto, CA, 94304. For newborns, please send any available documentation, including birth records or a hospital discharge summary. If you do not have enough time to arrange for transfer of these items prior to you appointment, you may bring them with you on your first visit.

Where to Go
The CFA Center is located in Clinic F of the Ambulatory Care Center at Lucile Packard Children's Hospital, 725 Welch Road, Palo Alto. When you arrive, go to the clinic information desk, located to the left of the main entrance.

During your Appointment
Your first visit to the center could last two to three hours, since you may need to wait for a procedure, for test results or to see members of the CFA Center staff.

We encourage both parents to be present at the initial evaluation to provide doctors with important genetic information and to learn about the specifics of your child's condition and care. During the evaluation, feel free to ask questions so we can clarify anything that is confusing or unfamiliar.

Members of the CFA Center staff will visit you and your child in an examination room. You will be instructed where to go if test or X-rays are needed. At the end of the morning, the CFA Center staff meet to discuss each patient's case. A staff member will tell you whether you should stay until the end of this meeting to speak with the team about the results of this collaborative discussion.

Follow-up Communication
The CFA Center staff remain in contact with you and your local doctor, dentist or other referring professional in order to provide results of your child's diagnostic evaluation and treatment. Please don't hesitate to call the center with questions.
